You don't realise how much you rely on each other and take each other for granted until you are no longer together. It's easy to feel alone and lost without that person who has been by your side for 5 years. I was emotional wreck and anything I did pushed my ex further away. Everything I thought was the right thing to do turned it to be very wrong! I'd suggest that you take your time to rehearse and decide exactly how you plan to approach your ex , know what your going to say and do and when so that you don't act irrationally! Don't call him unless he asks you too and don't follow him around like a puppy dog. Don't do anything extra or special like cooking. Most gestures will come across as you groveling. Don't try to kiss or cuddle, just be a friend. If he's interested, he'll make the next move. If something comes out of this, don't live with him right off. But beware, even after doing all these don't let him know about your desperation to get him back to your life.
